Dear Student or Parent:

The Sidney School District recognizes that the student activities program is an integral part of school life.  As such, this broad-based program should contribute to the intellectual, physical, social and emotional growth of students.  To this end, the District is committed toward operating all extra and co-curricular programs in a manner that not only teaches the techniques, fundamentals and strategies of the activity but enhance the social and emotional development of the participants and the spectators.

In an effort to provide students with activity programs consistent with the District’s goals, we are asking your assistance with the evaluation of the program that you are currently involved with OR just completed involvement with as a participant or parent.  The primary goal of any evaluative or assessment process is the improvement of the program and with the coaches/advisors working in the district’s activity programs.  Today’s coaches/advisors are required to meet a wide range of educational expectations and to help meet this new challenge, I am asking for your participation in the attached program assessment.

Completion of the ONLINE assessment will take approximately 10-15 minutes of your time, and by doing so, you will assist with the identification of program strengths and areas needing improvement.  In completing the assessment, please read the instructions carefully, since the format for response differs for each section.  You are encouraged to provide your name on the survey for follow-up purposes but this optional.  All responses will remain anonymous.   You may begin the survey by going to the following link:

In closing, I thank you in advance for your time and cooperation in responding to this request on behalf of the District and our student activities staff.

Sincerely,

Daniel T. Farr, Ed.D.
